Greenwich & Bexley Community Hospice has launched an Advance Care Planning (ACP) volunteer project to support people at the end of life in Greenwich to make their own unique care plans for the future.
The ACP volunteers will visit people up to four times over a one-month period in their homes to support them to produce an ACP.
Full training and support will be provided.

The ACP volunteer project, funded by Comic Relief, is a new initiative run by the Greenwich Care Partnership (GCP), an integrated community-based service led by Greenwich & Bexley Community Hospice, for people with life-limiting illnesses.
It provides care coordination, nursing and personal care to dying people at home.
