QUEEN Mary's Hospital is hosting an information day about aftercare tomorrow (September 26).
Patients and carers will be able to find out what support is available for patients once they leave hospital.
People will be able to speak to hospital staff about services such as:
- The Step Up,Step Down unit for people who do not need a hospital bed, but need extra care for a short period away from home.
- The bridging team, of nurses, occupational therapists and physiotherapists, which works with social workers and hospital staff to help ease the move from hospital back home.
- The virtual ward, which is a team of nurses who care for people who need daily support at home.
There will also be information about the district nursing teams, social workers, community health rehabilitation team, stroke services and paediatric services.
The information will be available in the reception area for Block B at the hospital in Frognal Avenue, Sidcup, between 2pm and 4pm.
