Lewisham and Greenwich NHS Trust is asking patients to nominate staff who have provided "outstanding care".

The Healthcare Hero award was set up to recognise NHS staff who have gone the "extra mile".

Chairwoman Val Davison said: "The Healthcare Hero award is a fantastic opportunity for us to celebrate the wonderful work our colleagues deliver on a daily basis. We have around 6,000 staff working at University Hospital Lewisham, Queen Elizabeth Hospital in Greenwich, Queen Mary’s Sidcup, and in a range of community settings in Lewisham.

"It’s really important to celebrate the fantastic work that goes on throughout the Trust every day, our Staff Awards including the Healthcare Hero award are a great way of doing that. So for staff that have gone the “extra mile”, help us congratulate them by nominating them for the Healthcare Hero award, and telling us how they made a difference."

Patients can use the online nomination form at http://www.smartsurvey.co.uk/s/WIJSQ/, email your story to lg.staffawards@nhs.net or write to FREEPOST LGT.

Nomination forms are available in patient waiting areas and main reception desks around the Trust at University Hospital Lewisham and Queen Elizabeth Hospital.

The closing date for nominations is September 7.
