Medical staff responsible for the care of a teenager who died under a train have been criticised in an internal report.

Veronica Romero, of Kelvin Gardens, Croydon, was killed on September 22 last year when she clambered onto the tracks at Wimbledon station.

She said "goodbye" over the phone to a friend, who heard a train's horn as it approached.

At a pre-inquest review at Westminster Coroner's Court yesterday, the court heard there had been an internal review carried out into Veronica's death at South London and Maudsley (SLAM) NHS Foundation Trust.

Her care co-ordinator and the care co-ordinator's manager are both criticised in the report.

The care co-ordinator is now on long term sick leave from SLAM and the manager no longer works there.

Coroner Dr Fiona Wilcox said: "Her care co-ordinator has been criticised on the report and it is conceivable that she could be criticised in the inquiry.

"I would want her before me and I would want her participating in court.

"That's to protect her Article 6 rights and to ensure that the enquiry is as full as needs be."

Veronica's brother Juan said he does not blame the staff at SLAM for what happened.

Addressing the coroner he said: "I would not put the blame on the people.

"We only know what happened because of the person she was on the phone to telling us.

"It all happened within two or three hours."

He told the court she had previously tried to take her life at the same station and occasionally did not take her medication because of how it made her feel.

In the days before the 18-year-old’s death she had been supporting a friend whose father had recently committed suicide.

They had been best friends in the past and Veronica was trying to get back to this again.

Veronica, a former pupil of Carshalton High School for Girls, quit an arts course at Carshalton College and had started working at Rift and Co bar in Sutton High Street shortly before her death.

She was a talented amateur actress and a member of the award-winning Carshalton youth theatre group Savvy.
